Limes Supported Living
The Limes is a community supported living service providing care for people with diverse and complex needs living with a learning disability and/or autism. The service aims to support people to move from hospital, residential care or family homes to live in their own accommodation whether this be on their own or with friends. We are situated in the peaceful village of Mountsorrel, just south of Loughborough.
The scheme consists of 6 brand new, spacious self-contained 1-bed flats, all of which have been completed and decorated to a high standard including; luxury kitchen with integrated appliances, lounge, bathroom with bath and shower and views out to the landscaped gardens.
The key factor in our model of care is progression with continual assessment of needs and individual objectives empowering people to live as independently as possible.
Regular review meetings involving the service user, their family and professionals are held to review outcomes, set new goals and re-assess support levels as individuals progress.
Our care model uses outcomes measures, risk assessment frameworks and holistic assessment tools to establish the fundamental needs of the person’s characteristics and behaviours which are then input into our integrated electronic records management system – Person Centred Software. This enables us to provide comprehensive reports on individual progress against the key personal targets on actions taken, support given, as well as providing strategic management information on the key outcomes achieved.
Our service user’s profile:
• Adults from the age of 17-65
• Individuals with a primary diagnosis of learning disability and / or autism

Location
The Limes is situated in the peaceful village of Mountsorrel in North Leicestershire. The flats are in a pleasant residential area within a mile from all local amenities including supermarkets, a library, shops and cafés with easy access to public transport routes serving Loughborough and Leicester.
The village is served by two medical practices and a pharmacy, as well as an opticians and dentist.
